Filing a civil rights complaint
If you think we failed to offer these services or discriminated based on race, color, national origin, age, disability, or sex, you can file a civil rights complaint, also known as a grievance.
If you have a complaint that is not related to civil rights, please call the Member Services number listed on the back of your membership ID card.
You can file a civil rights complaint with our Section 1557 Coordinator in writing to Section 1557 Coordinator, 233 S. Wacker Dr, Suite 3700, Chicago, IL 60606. Email: Section1557Coordinator@Carelon.com.
Civil rights complaint forms are also available at the U.S. Department of Health and Human Services.
